Your Individual Rights (HIPAA)
As a patient, you have the right to:
- Access: Inspect or receive a copy of your health records.
- Accounting: Receive a list of instances where we disclosed your PHI for reasons other than treatment or operations.
- Amendments: Request corrections if you believe information in your record is inaccurate or missing.
- Restrictions: Request (in writing) that we limit how we use your PHI. While we will consider all requests, we are not legally required to agree to every restriction.
Dignity: Be treated with respect regardless of race, color, religion, sex, age, disability, or sexual orientation.
